Hypertension & diabetes screening and follow-up for community health workers
CB HEARTS Tool helps community health workers (CHWs) screen, manage, and follow up patients for hypertension and diabetes care at the community level. Built for the field, not the office: • Quick eligibility screening — capture blood pressure, blood sugar, urine protein, and relevant medical history in a few taps to decide program eligibility on the spot. • Telemedicine visit forms — for both new and returning patients, with structured fields for symptoms, medication, and lab results. • QR-based patient lookup — scan a patient's card to instantly pull up their record on a return visit, no re-typing required. • Automatic PDF visit summary — every visit generates a shareable PDF report. • Works offline — submissions are saved on the device and sync automatically once you're back online. • Clinic-based accounts — each health worker is linked to their assigned community clinic, verified through a simple admin approval step. • Admin dashboard — administrators can approve accounts, manage community clinics, and monitor submissions across all clinics from one place. CB HEARTS Tool is developed for NHF's community-based hypertension and diabetes care program and is intended for use by trained community health workers and program administrators.
