Your phone writes the daycare incident report, and the parent signs it before they walk back out.
Type what you saw, mark it on a body map, attach photos, then turn the phone around for a signature. Out comes a PDF to send home or keep on file. The same app runs the morning arrival check, so a bruise that arrived with the child is written down before the day starts.
WHAT GOES ON THE FORM
A first name, the time, the room or the yard, what you saw in your own words, who else saw it, what you did next, and how the parent heard - phoned, texted, told at pickup, not yet, or could not be reached. Another child involved prints as "another child", and you can show a last initial only.
A BODY MAP, NOT A PARAGRAPH
Tap the front or back figure to drop a numbered mark, then drag it onto the scrape. Each mark takes a label, a size in centimeters or inches and a note, and the chips suggest only words you typed yourself - this app ships no starter vocabulary for a child's body. Six photos a record live in the app's encrypted storage, not your camera roll, each pinnable to its mark.
THE MORNING ROUND, EVERY CHILD
An incident report is the page nobody wants to open; the daily health and arrival check you fill every morning. The Today tab counts who is checked in, and one tap on a name logs an arrival with nothing to note. Per child, turn arrival time, body map, photo, parent note and guardian initials on or off.
THE SIGNATURE IS TAKEN ON YOUR PHONE
Hand-off mode shows that child's record and nothing else; your PIN is the way back if you set one, and where the device allows it the app pins itself to that screen. A parent who would rather not sign is recorded as that, with a note in your words, not a blank line. Printed labels switch to English, Spanish or French per guardian; your own words stay as you typed them.
SIGNED MEANS CLOSED
Once signed, the record is sealed and its footer carries a record stamp, so the page in a parent's hand and the file on your phone match. Anything you learn later goes on as a numbered addendum with its own date and signature. Medication is paperwork only: photograph the signed authorization, type the medicine as written, set an end date, and each dose adds a time-stamped line naming who gave it.
WHAT LEAVES THE APP
- The incident report as a PDF: your header and logo, the marked body map, the photos, both signatures
- Everything on one child over a date range, in one document
- A CSV log of date, child, location and signature status when a licensing visit asks
- The Android print dialog when paper is wanted
WHAT THIS APP DOES NOT DO
It keeps what you observed and typed. It does not rate how serious anything is, propose treatment or first aid, say when to call anyone, or decide whether a child goes home.
NOTHING LEAVES THIS PHONE
No account, no sign-in, no server of ours, no advertising, no analytics library. Records live in an encrypted database on the phone; turn on a six-digit PIN lock in Settings, with fingerprint as a shortcut. Back everything up, photos included, to one file in a folder you pick, locked with a passphrase kept elsewhere. Enter the years your licensing agency asks you to hold records, or leave it empty and nothing is removed.
WHO IT IS FOR
Home daycare and childcare centers, family child care, preschool and toddler rooms, after-school programs, summer camps, a nanny or a babysitter - anyone filling in daycare forms, a child accident report form or a boo-boo report by hand.
PURCHASE
A weekly or yearly subscription, or a one-time lifetime purchase, unlocks the app. Price and billing period appear before you buy.
